Because the Delaware Estate Tax was repealed for individuals dying on or after January 1, 2018, this return is not required for 2018. If you have an estate involving a decedent who died prior to January 1, 2018, please use the 2017 form links below.

Any printed reproduced forms that have crucial inconsistencies with the official version which results in an unprocessible tax return will be returned to the taxpayer. The taxpayer will be notified that their tax return cannot be processed and the taxpayer must resubmit the tax return using an approved reproduction of the form. There is no perfection period for paper forms.

  • In order to apply for this program you must meet all of the following eligibility requirements: You have incurred a delinquent tax liability that has resulted in an assessment.

  • You have declined to pursue or exhausted all avenues of administrative appeal. You cannot make an offer in compromise if there is any issue currently under appeal.

  • Two years must have passed since you became liable for the tax.

  • You must have filed all returns required to be filed with the Comptroller's Office or have been assessed by the Comptroller's office for not filing a required tax return.

  • You must not be currently involved in an open bankruptcy proceeding.

  • For taxes owed by a business or the officers of the business, the business must be closed.

  • You are unlikely to be able to make payment in full any time in the foreseeable future due to your financial situation. You either are without resources or unable to apply present and/or future resources to paying the outstanding tax liability.

To request a copy of a Maryland tax return you filed previously, send us a completed Form 129 by mail or by fax. Please include your name, address, Social Security number, the tax year you are requesting and your signature. If you are requesting a copy of a joint return, include the information for both taxpayers and their signatures.

Please include your business name, address and your nine-digit federal employer identification number (FEIN) and eight-digit Maryland Central Registration (CR) number on your returns and correspondence.

ELECTRONIC FILING MANDATE:In accordance with changes signed into law in June of 2022, a larger business registrant will be required to use electronic means to file returns and remit taxes to the State of Rhode Island for tax periods beginning on or after January 1, 2023.

NEW SALES TAX FORM:Starting with the return for the period ending January 2023 [filed in February 2023], monthly and quarterly Sales and Use Tax filers will begin using the RI Division of Taxation's new sales form - Form RI-STR. A Social Security 1099 or 1042S Benefit Statement, also called an SSA-1099 or SSA-1042S, is a tax form that shows the total amount of benefits you received from Social Security in the previous year. It is mailed out each January to people who receive benefits and tells you how much Social Security income to report to the IRS on your tax return.

If it seems nothing happened after you clicked Download, or you don't know where your browser saved the download, you can find a list of downloads using the keyboard shortcut Ctrl + J in Internet Explorer, Chrome or Firefox. This will open the downloads location and allow you to choose the TaxAct installation file. You can also use Windows search to locate the installation file on your computer. TaxAct installation file names begin with "taXX" (where XX is the last two digits of the year).

If you click on the installation file and get an error, this could be because the download is not complete. Incomplete downloads may be due to internet connection problems. Please confirm the download is complete by checking your browser for any messages showing the progress of download. Alternatively, you can try using a different browser.

Members living in states with laws that require reporting of health coverage will continue to receive a paper copy of the Form 1095-B for state filing tax purposes. Subscribers filing taxes in one of these states (such as NJ or DC) are encouraged to retain a copy of their 1095B for their state tax records. If you are filing taxes for an individual mandate state and do not have a copy of your 1095B, you may download one immediately from your member website or request one by calling the number on your ID card or other member materials. By retaining a copy of the 1095B form, you will be prepared for any questions about your state return. If you are unsure whether your state has an individual mandate, check with your state revenue department or a tax professional.

Most fully insured UnitedHealthcare members will no longer automatically receive a paper copy of the Form 1095-B due to a change in the tax law. Under new law that became effective beginning with the 2019 tax year, the IRS penalty for not having health coverage was reduced to zero. Therefore, individuals no longer need the information on the Form 1095-B to file a federal income tax return. Members living in states with laws that require reporting of health coverage will continue to receive a paper copy of the Form 1095-B for state filing tax purposes.
